What is rhinoplasty surgery?

Rhinoplasty is a surgical procedure which can alter the size, shape and profile of your nose. It is commonly referred to as a ‘nose job.’

During surgery, bumps / humps can be removed; a deviated nose can be straightened; a broad nose can be narrowed; and nostrils can be resized or reshaped. The tip of the nose can also be refined and reangled.

The procedure involves removing any excess bone and / or cartilage and reshaping the remainder.

The key types of rhinoplasty include:

Closed Rhinoplasty: This is the most common type of rhinoplasty, as it is less invasive, yet can deliver significant results.

All the incisions are hidden inside the nose and the surgery is performed inside the nostrils, preventing any visible scarring.

This surgery is generally completed faster than open rhinoplasty and there is less nasal tissue irritation, which means healing and recovery time is usually shorter.

Open Rhinoplasty: Also known as external rhinoplasty, this type of surgery is more invasive and is often used for patients who require more extensive work on their nose.

This technique is usually suited to patients who have structural nasal issues or who have suffered severe trauma to the nose.

Tiny incisions are made both inside the nostrils and on the fleshy column between the nostrils (the columella). This enables the surgeon to lift the skin from a large portion of your nose, allowing access to the inside of the nasal cavity, so more significant changes can be made to the structural parts of the nose.

Revision Rhinoplasty: Revision, or secondary rhinoplasty, applies to any patient who has previously undergone a rhinoplasty procedure, but requires improvement to the appearance and / or function of the nose.

It is sometimes performed with septal surgery (the straightening or reshaping of the central cartilage and internal bony framework of the nose) and may use either the open or closed technique.

Non-Surgical Rhinoplasty: As the name suggests, this type of procedure is non-surgical and is performed using injectable filler (usually hyaluronic acid).

The filler is injected underneath your skin to smooth out the appearance of the nose and address any minor aesthetic issues.

This procedure is usually completed within 15 to 45 minutes with no downtime required.

Unlike traditional rhinoplasty, the results of non-surgical rhinoplasty are not permanent, generally only lasting between 6 and 18 months.

Closed rhinoplasty is generally far less invasive than open rhinoplasty. This often means a shorter procedure time, less swelling and a speedier recovery.

During a closed rhinoplasty procedure, the incisions and surgery are performed inside the nostrils.

Closed rhinoplasty is usually recommended for patients who only require small structural changes to their noses.

Open rhinoplasty, on the other hand, is performed from outside of the nose. A small incision is placed on the tissue between the nostrils (the columella), in addition to the incisions inside the nostrils – allowing greater visibility and access to the entire nose structure. This can result in a tiny hairline scar between the nostrils, however it is often barely visible.

This method is often used for more complex cases that require a more significant reshaping of the nose.

This is an essential part of pre-surgical planning and every care is taken to ensure you are fully informed and your surgeon has a solid understanding of your requirements.

Your nose will be meticulously examined by your surgeon, who will address factors such as whether the nose deviates to the left or right; the placement of the nostrils and the overall structure of the nose.

They will also consider your other facial features and review your full medical history.

If you're a candidate for surgery, your surgeon will develop a customised plan for you.

Photographs will be taken before surgery to help with preoperative planning and to compare with postoperative results.

Anaesthesia: Patients are usually put under general anaesthetic (fully asleep), but in some cases they may be put under twilight sedation.

Surgery: As a general rule, in addition to the surgeon and the anaesthesiologist, a nurse and a surgical technician will also be present during your surgery.

The surgeon will make a small incision inside your nostrils – as well as through the base of your nose (the columella) if you are undergoing an open nose job.

They will then sculpt the bone and cartilage underneath your skin to shape your nose. If more cartilage is needed, it can often be taken from deeper inside your nose, or from your ear.

Once the nose has been re-sculpted, your surgeon will replace your skin and tissue over the cartilage and then stitch up the incisions. They will then tape your nose and place a splint on the outside of your nose to protect it and reduce swelling as it heals. Splints or dressings may also be placed inside your nostrils and a ‘drip pad’ (a small piece of gauze held in place with a strip of tape) may be placed under your nose to absorb drainage of mucus and blood.

Rhinoplasty surgery generally takes between 1.5 and 3 hours.

Following your surgery, your nose will likely be congested due to swelling and / or from the splints / dressings placed inside your nostrils. The splint, dressings and stitches usually remain in place for up to 7 days.

Internal swelling of the nose usually subsides within a few weeks, however it may take up to 12 months for you to see the final results of your surgery.

Slight bleeding and drainage of mucus and old blood are common for a few days post-surgery, or after removing the dressing.

You may also notice some swelling and bruising around your eyes and cheeks, however this should subside within 5 to 10 days. The use of icepacks can help to reduce swelling.

Rhinoplasty recovery time is generally 1 to 2 weeks, but the amount of downtime required is ultimately up to the patient.
